I hereby grant the Born To Be Wealthy Foundation permission to use images, videos, and likenesses of my child/children listed above in the following manner:

1.⁠ ⁠Use of Media: The images, videos, and likenesses of my child may be used on the Foundation's website, social media platforms (including but not limited to Facebook, Instagram, Twitter), promotional materials, newsletters, press releases, and any other medium used for the promotion of the Foundation's activities and objectives.

2.⁠ ⁠Purpose of Use: The media may be used for the purposes of promoting the Foundation's events, fundraising activities, educational programs, and awareness campaigns aimed at supporting the Foundation's mission.

3.⁠ ⁠Duration of Consent: This consent is given indefinitely until expressly revoked in writing by the undersigned. I understand that I may withdraw consent at any time by notifying the Foundation in writing, however, I also understand that due to the nature of the internet and printed materials, it may not be possible to remove all instances of my child's likenesses once they have been published.

4.⁠ ⁠No Compensation: I understand that the submission of my child's image, video, or likeness to the Foundation is without any compensation or expectation of compensation. The Foundation's use of the said media shall not constitute employment, imply an agency or partnership relationship, or result in any royalties or other form of compensation.

5.⁠ ⁠Release of Liability: I hereby release, discharge, and agree to hold harmless the BTBW Foundation, its representatives, employees, volunteers, agents, successors, and assigns from any claims, damages, or liability arising from or related to the use of my child's image, video, or likeness as consented to herein.

By signing below, I affirm that I have read this consent form in its entirety, understand its contents, and agree freely without any reservation.
